End-of-life care may take place in various settings—such as the patient’s home, a hospice facility, hospital, or nursing home—and often includes palliative and hospice services. Additional Information about Medicaid Recipient

If you are a Medicaid recipient interested in home care services, please download and print the DMA 3051 form linked above.


Please take this form to your physician to complete. Upon completion of the form, your physician will fax the form to Liberty Healthcare Corporation of North Carolina. Your application will be screened. You will receive a call to schedule an appointment to be assessed by a case manager to determine if you qualify. If approved for services, you will receive a defined care plan which you cannot deviate from unless there is a status change. The care plan is reviewed every quarter by our RN staff at Compassionate Hands Home Care. If you need changes, we will assist to make sure you receive the level of service you need. Most Medicaid recipients can receive somewhere between 5 to 20 hours per week. However, the hours of care one receives are based on the assessment.


If you are interested in private pay services, please contact us. We will schedule an appointment to meet with you to discuss the days of the week, hours requested, condition of client, level of care assistance needed, and if any other services are needed in addition to activities of daily living (such as errand assistance, light housekeeping etc.) to determine cost.
